PEBB Statewide, Providence and Moda Medical Plans
Vision insurance is provided by VSP:
- VSP Plan Summary provides benefit coverage details
- Receive care from a VSP provider--you pay the applicable co-pay and the provider bill VSP directly
To find a provider or retail chain affiliate, visit VSP or call 800-877-7195 - Choose to see a provider of your choice not in the VSP network--you pay for services and submit a Reimbursement Form directly to VSP for payment
Kaiser Medical Plans
Vision coverage is provided as part of enrollment in a Kaiser Medical Plan and services must be received from a Kaiser provider at a Kaiser medical facility.
Benefit details are available in the Kaiser Medical Plan Summary applicable to the plan elected.
